NOTIFICATION OF DISENGAGEMENT FROM LOCAL GOVERNMENT SERVICE REQUEST FOR MY RETIREMENT BENEFIT

 

I, AHMED ABDULAZEEZ of the above named Local Government wish to inform you of my disengagement from the service and also request for the payment of my retirement benefit.

 

I am a Deputy Director (Finance) Grade Level 16, and was employed to the Local Government Service on the 10thSeptember, 1990 and wish to disengage from the service with effect from 1stDecember,2021.

 

Thank you for the privilege given to me to serve the local Government for the period of my service and hoping that my application will be given speedy approval.
